Our internal medicine doctors focus on adult medicine and serve as your first point of contact in our medical practice. Our providers have great expertise in preventive care and are specially trained to manage complex problems.
Appointments
We accept new patients and most all insurances. We prefer to see our patients on an appointment basis, and we request that you call in advance so we can reserve time for you. Same day appointments are available. Routine appointments may be scheduled in advance.
When Visiting Our Office
To ensure your office visit goes as smoothly as possible, please bring the following items with you:
First Visit
Driver’s license or other photo ID
Insurance card and co-pay or co-insurance due
Medication list (please include over-the-counter medications)
Completed New Patient Registration form (download above)
Completed medical release form if applicable
Every Visit
Insurance card and co-pay or co-insurance or deductible due
Medication list (please include over-the-counter medications)
List of medications requiring refills
Schedule an appointment:
To make an appointment, give us a call. Please remember:
- Arrive 15 minutes prior to your appointment time to complete a few forms.
- Bring your insurance card and photo ID (such as a driver's license).
- Bring all medications you are currently taking.
- If you must cancel your appointment, please notify us 24 hours in advance.
Fees and insurance
Patients are responsible for understanding insurance coverage benefits. Please verify that we are listed as approved providers prior to scheduling your appointment with us.
- We accept most insurance plans.
- Payment is due at the time of service.
- We accept cash, checks, debit, and credit cards.
Special Forms and Correspondence
processing fee will be applied for special forms and correspondence, including but not limited to requests related to life insurance, employment, sports/physicals, disability, driver’s licenses, nursing home and assisted living admission forms, and FMLA.
Holidays:
We are closed so our team can enjoy the following holidays with their families. However, we continue our 24-hour availability during holidays for urgent medical needs through our answering service.
